Forms:

To avoid inconvenience and delay in processing, please download the required form(s) and complete them before coming to the Embassy. Make sure you bring the completed forms and supporting documents on the day of your appointment. Applicants with incomplete or no forms will be requested to reschedule.

Passport Fees

Form of payment is now only through Pay.gov at https://www.pay.gov/public/form/start/1274042472/

Applicants using a DS-82:

To renew an adult passport should use the mail-in renewal program and pay the fee online – it’s convenient and easy.  Paying online allows you to mail your application to the U.S. Embassy or Consulate without the need to pay in person.  Qualifying adult passport renewal applicants may pay the passport renewal fee online via U.S. or international credit or debit card, electronic funds transfer from a U.S.-based bank account, Amazon Pay, or PayPal.  Find out more here by clicking “How to mail-in your adult passport renewal from anywhere in Pakistan.”
