Legal Name Change

There are two ways to apply to change the legal name in your U.S. passport. The application you use depends on certain factors.

If Your Current Passport is Less than One Year Old:

If the change of name is requested within one year after issuance of your most recent passport, submit application form DS-5504 as well as the documentation required below. There will be no charge for routine processing in this case.

Please submit the following:

  • Two U.S. passport sized (2x2inch) photographs.  See photograph requirements for information on the correct size.  Photographs which do not meet the requirements will be rejected.
  • Your most recent U.S. passport
  • Application Form DS-5504 (If you do not have a Social Security number, enter 000-00-0000)
  • Documentary evidence such as a court order, marriage, divorce certificate, or other satisfactory evidence to support a change of name. Only originals or certified copies of these documents will be accepted.

If Your Current Passport is More than One Year Old:

If the change of name is requested more than one year after issuance of the current passport, submit the required documents below.  You will be required to pay the regular passport fee in this case.

Please submit the following:

  • Your most recent U.S. passport
  • Two U.S. passport sized (2x2inch) photographs.  See photograph requirements for information on the correct size.  Photographs which do not meet the requirements will be rejected.
  • DS-82 passport form (fill out online and print).  Complete the form online, and print it.  Please DO NOT mail the form to the United States or the U.S. Consulate.
  • If you have changed your name since your previous U.S. passport was issued, you will be required to furnish evidence of your name change, such as marriage certificate or court order.
  • Fees: Fee is payable in cash or by credit card.
  • Incomplete, unsigned or incorrect applications will not be accepted and will delay processing.
